We are holding a Covid-19 Vaccination event at Surprise Lake Middle School on April 16, 2022. Second doses will be automatically scheduled on May 14, 2022.
We will administer the following vaccines (PRIME SERIES AND BOOSTERS – 3rd AND 4th doses per cdc guidelines (BOOSTER FOR 12+):
-Pfizer for 5+
-Moderna 18+
-Johnson & Johnson 18+ (We will administer MRNA boosters only for J&J doses)
-TDAP Vaccines for 12-18 YEars
We will provide 1st, 2nd, 3rd and 4th Covid-19 doses (per CDC/FDA guidelines).

Please arrive 10 minutes prior to your appointment in the assembly area.
Please bring your driver’s license (or parent’s/guardian’s) and insurance card. You do not need to bring an insurance card if you do not have insurance.
