Be sure to complete all required boxes. Press submit, and you will be all set! Please read below about appointment selection process.

Appointment Availability

We are offering pop-up clinics through the Puget Sound region, so please check on the registration page (or the top menu bar) for those events.  We are also booking appointments live in our Proctor-based clinic.

We are currently administering the 2024/2025 version of Moderna, Pfizer, as well as a number of other adult vaccines (flu, singles, RSV) and a variety of childhood vaccines.

Omlife’s Providers will medically evaluate when the following risks are identified during the screening process: 1) contraindications to the vaccine 2) if you have multiple co-morbid conditions (diabetes, hypertension, COPD, Cancer etc….), and/or 3)if you are not feeling well or have a fever. We will take your vital signs. The providers will review you past medical history, allergies, and medications.

After you are vaccinated, you are no longer required to be observed for 15 minutes; however, we typically recommend that you stay for at least 5 minutes.
