WE HAVE FLU SHOTS, BIVALENT COVID-19 BOOSTERS AND OTHER VACCINES. PLEASE USE THIS FORM TO REGISTER FOR ANY OF THESE DOSES.

We are holding a Covid-19 Vaccination event at Curtis Junior High School on November 4, 2022 starting at 3:30 pm.

We will administer the following vaccines (PRIME SERIES AND  BIVALENT (omicron) BOOSTERS – 3rd AND 4th doses per cdc guidelines (BOOSTER FOR 5+):

-Pfizer (Bivalent – 5+ and monovalent Prime/Booster series 5+)

-Moderna (Bivalent and Prime series) 18+

-Novamax 12+

-TDAP and other childhood Vaccines for 12-18 YEars

-shingles and pnuemococcAL and other ADULT Vaccines

-Flu Vaccines for 5-64 years (click for info)

-Flu Vaccines for 65+ YEars (click for info)

We will provide 1st, 2nd, 3rd, 4th and 5th doses Covid-19 doses (per CDC/FDA guidelines).

All Flu VACCINES for 5-64 years of age are FREE FROM eggS AND AnTIBIOTICS.

Please arrive 10 minutes prior to your appointment in the assembly area.

Please bring your driver’s license (or parent’s/guardian’s) and insurance card.  You do not need to bring an insurance card if you do not have insurance.
